For over three decades, my journey as an optometrist, professor, and scientist has been deeply intertwined with the lives of those affected by keratoconus. I have had the privilege of managing more than 5,000 keratoconus patients, spanning every age group and every stage of this complex condition — from the mild, early forms to the most advanced, vision-threatening cases.

This book is not just a compilation of clinical facts; it is a culmination of 36 years of lived experience, compassion, challenges, and triumphs. I’ve witnessed patients break down in despair, only to rise again with renewed hope after the right diagnosis and personalized care. I’ve seen the confusion on a teenager’s face when told their vision loss wasn’t fixable by glasses, and the joy when their world became clear again through a custom scleral lens. I’ve sat beside patients who had given up on their careers, relationships, and even their confidence — and helped them rebuild their lives.

Over the years, I’ve embraced not only non-surgical modalities like custom scleral lenses, high-order aberration correction, and impression lenses, but also surgical solutions such as CTAC, CAIRS, CXL, and IPCL when the condition demanded it. This book reflects that complete spectrum — from the first sign of blurred vision to advanced surgical rehabilitation.

I am immensely grateful to Dr. Quresh B. Maskati, the internationally acclaimed corneal surgeon, who has been an instrumental guide in my journey and has kindly agreed to contribute as a co-author of this book. He has operated on many of my patients with skill and dedication, helping them achieve remarkable outcomes. At times, when further expertise was needed, we have referred our patients to other renowned surgeons across India and the world, including Dr. Kasu Prasad Reddy  and internationally to experts such as Dr. Rajesh Khanna in California and Dr. Arun Gulani in Florida. Their contributions and surgical excellence have significantly expanded the scope of care available to our patients.

I could become an expert in scleral lens fitting because of the mentorship and knowledge shared by legends like Ken Pullum (UK), Dr. Donald Ezekiel (Australia), and Dr. Perry Rosenthal (USA). Their pioneering work and generous guidance shaped my understanding of the optical, anatomical, and emotional aspects of managing keratoconus with advanced contact lens technologies.

My intent in writing "Keratoconus: Understanding, Managing, and Thriving" is to empower patients, eye care professionals, students, and families with practical knowledge, compassionate guidance, and the reassurance that keratoconus is manageable, treatable, and above all — not the end of your vision journey.

You’ll find here not only scientific explanations but also case stories, myths and facts, answers to real-life patient questions, mental health support, and the day-to-day challenges patients face — socially, financially, and emotionally. You’ll also meet patients who have succeeded — children who returned to school, adults who resumed work, and elderly patients who regained independence.

This book is written in a friendly, empathetic voice — the voice I’ve used with every patient who sat in front of me confused and scared. Myopia, once seen as a minor inconvenience that could be managed with glasses, has now become a global epidemic. Every year, I meet more children whose numbers rise faster than their parents can replace their spectacles. Behind those numbers lies something bigger — the risk of lifelong complications, the emotional struggles of children, and the heavy burden on families and healthcare systems.

This book, Shaping Young Eyes: A 39-Chapter Journey into Myopia Control, is born out of my 36 years of clinical practice, research, and collaboration with mentors and colleagues across the world. For more than three decades, I have had the privilege of working at the intersection of science, medicine, and artistry—helping patients regain not just the appearance of their eyes, but also the confidence and dignity that come with it. The Eye Reflections: The Art and Science of Artificial Eyes & Prosthetic Contact Lenses is the culmination of that journey.

The human eye is more than an organ of sight; it is a mirror of identity, emotion, and self-expression. When an eye is lost or disfigured, the impact extends far beyond the physical—it reaches into the very core of a person’s self-esteem and human connection. The art of crafting ocular prostheses, therefore, is not only a scientific discipline but also a profoundly human one.

A Brief History of Artificial Eyes

The desire to restore the appearance of the eye is as old as human civilization. Ancient records and archaeological findings show that people once used gold, gemstones, and natural substitutes to overcome the stigma of ocular disfigurement.

In India, references to ocular prostheses and transplant-like procedures can be traced back as early as 5600 BC in the Garbhopanishad. Classical physicians such as Sushruta, Charaka, and Vagbhata pioneered surgical techniques and instruments for eye and facial reconstruction, long before similar advances were recorded in the West. Acharya Prashastpad of the Vikramshila dynasty was even honored with a golden eye trophy in the 7th century for his contributions to eye surgery.

In the western world, one of the earliest known prosthetic eyes—dating back to 2800 BC—was discovered at Shahr-i Sokhta in Iran, fashioned from bitumen and gold to imitate an iris. Centuries later, Europe advanced the art of glass eyes, with Germany leading production until World War II, when shortages spurred the transition to plastic.

Modern ocular prosthetics took shape with the adoption of medical-grade PMMA—biocompatible, durable, and life-like. Today, custom-made artificial eyes provide superior cosmetic and functional results compared to mass-produced stock eyes, which are still used in many parts of the world where access to skilled ocularists is limited.

The Present and the Future

Through my years at Shekhar Eye Research and collaborations with surgeons and ocularists worldwide, I have seen how far we have come—and how much further we can go. Advances in impression techniques, scleral prosthetics, and custom-tinted lenses now allow us to deliver results once unimaginable. Yet, at its heart, this work remains deeply personal: listening to each patient’s story, restoring their appearance, and, with it, a piece of their identity.

This book is my attempt to bring together history, science, artistry, and humanity. It is written not only for ophthalmologists, optometrists, and ocularists, but also for patients, families, and anyone seeking to understand this remarkable field. My hope is that these pages serve as both a guide and a reflection—where science meets compassion, and art restores dignity.

My father, Vasant Yashwant Chawan, though an engineer by profession, carried within him a quiet yet profound spirituality. In our home, the laws of physics and the laws of karma were not separate — they were two perspectives on the same eternal truth.

This upbringing planted in me a deep conviction: that ancient wisdom and modern science are not rivals, but companions. Over the decades, my professional journey as a physicist, optometrist, researcher, and inventor took me from advanced medical laboratories to international conferences, from the frontiers of space exploration to the intricate mysteries of quantum physics. Yet, everywhere I looked, I found reflections of the same truths I had heard in my mother’s kirtans and my father’s quiet teachings.

5th Dimension is the result of years spent studying both the timeless Vedic scriptures — the Vedas, Upanishads, Bhagavad Gita, and Puranas — and the works of great scientific minds from Carl Sagan to modern quantum theorists. It is an attempt to weave these threads together into a single tapestry, accessible to anyone who seeks to understand the deeper dimensions of life.

In these pages, we will explore realms beyond the five senses, the nature of thought as a timeless traveler, the possibility of life beyond Earth, and the ways in which our consciousness expands in harmony with the universe itself. We will consider not only the spiritual and philosophical aspects of these truths, but also the medical, genetic, and evolutionary implications for the future of humanity.

My hope is that this book will not only broaden your understanding of the cosmos but will also encourage you to look within — for in the end, the greatest journey is not across space, but into the infinite dimension of your own consciousness.
